Just in time for us, the National Alliance to End Homelessness has released a multi-stage coordinated assessment tool.  Check out a discussion and link to the tool here.

Locally, we have been working toward use of the the VI-SPDAT, a tool for identifying and prioritizing households for permanent supportive housing.  This is a great tool for use at the back door of homelessness–evaluating what people need to get them out of homelessness.

The newly-released NAEH tool adds front-door assessment to the tool, meaning that it helps to assess households on the verge of homelessness and about to enter shelter, to determine whether diversion/rapid rehousing is appropriate.  The tool also incorporates HMIS universal data elements.

As we move forward in our coordinated assessment conversations, expect to be looking more closely at the NAEH tool.
